<p>What is the Community Integrated Paramedicine Program?</p>
<p>Leaving the hospital with a chronic illness or other serious condition? Consider Northern Arizona Healthcare’s Community Integrated Paramedicine Program, an in-home care service designed for you. You will need a referral from Northern Arizona Healthcare Care Management to participate in the program.</p>
<p>Certified paramedics from Guardian Medical Transport visit you in in your home after you leave the hospital to help make sure you are feeling better and understand your plan of care.</p>
<p>These paramedics can assess your:</p>
<ul>
<li>General health.</li>
<li>Personal and family health history.</li>
<li>Emotional wellness.</li>
<li>Medication issues.</li>
<li>Home safety.</li>
<li>Chronic wounds and wound care.</li>
<li>Chronic illnesses, including:
<ul>
<li>Congestive heart failure.</li>
<li>Chronic Obstructive Pulmonary Disease, or COPD. </li>
</ul>
</li>
<li>Asthma.</li>
<li>Sleep apnea.</li>
<li>Diabetes.</li>
</ul>
<p>Just as importantly, program paramedics serve as a link between you and your doctor – and fill the gap in care that sometimes happens when you are transitioning from the hospital to your home.</p>
<p>The paramedics can act as translators for medical questions you have, so you understand the answers. Finally, they can help you access further community resources. </p>

<p>Not exactly what I'm looking for, but so much better than having an LMRFD paramedic / EMT tell you that you're not going to get any drugs from them and that you should drive yourself to the hospital (happened to a friend in Meadview in 8/18).</p>
